Most likely it was warped on installation (welding or delivered that way) or compressive/bending loading due to foundation settling.
Facade inspection. Any thoughts on what is causing this steel lentil to warp? We have been inspecting buildings for 20 years and have never seen this before. It is all over the building, all sides including north facade so we have ruled out thermal expansion. It clearly wasn't built like that due to the gap that formed between the steel and the mortar. This is an apartment building built in the 1980s. Tried to upload the pic, I hope it worked. LOL

Most of my replies on other boards are that it is water freezing and bending the lintel. No weep holes, Michigan.
Frozen water? Sure, that's a possibility but how did the water get there in the volume needed to expand and warp the lintel?
