When we checked the broach tool we only see the damage on chamfer (45 degree). It can be a reason for this failure mode?

It can be a microstructure of material you have to check it.
It's hard to draw a lot off the picture, but the surface looks like its been galled. I would check the relief angle on the rougher and finisher teeth. I would also check the gullet of the broach to insure you have enough space for the chip being pulled by he tooth.
Other checks to insure you are starting off correctly:
- Insure the pitch of tooth form allows you to have 2 teeth engaged through out the cut. With out this you can't control material removal rate. This also causes broken teeth and poor finishes.
- Insure finisher teeth have zero rise after 1st tooth.
